AFC - Shared Living Case Manager

POSITION DESCRIPTION: The Case Manager is primarily responsible for assessing the needs of individuals supported and ensuring placement with appropriate caregivers who have the ability and desire to provide quality care and supports. Ensuring ongoing placements are consistently meeting expectations. Providing advocacy, education and support for individuals and caregivers. The Case Manager may carry a case load up to 15 to 30 AFC members.
