Position Summary

To deliver comprehensive and individualized case management services grounded in Trauma-Informed practices. Responsibilities include facilitating client access to essential housing, employment, and benefit resources, as well as coordinating services with community, behavioral health, and legal partners to support client stability and long-term self-sufficiency.



Essential Duties

  • Conduct a case management services plan within the first week of admission and document all findings in UWITS.
  • Developing discharge plan at admission to support clients’ transition into the community and coordinating service connection and warm handoffs.
  • Assist clients with securing housing, employment, and public benefits as appropriate
  • Attend weekly treatment team meetings to provide updates on client progress, identify challenges, and support the alignment of treatment plans and case management goals.
  • Meet individually with each client once per week to review progress on their client-centered treatment plan and case management goals.
  • Ensure clients are meeting legal obligations and submit monthly progress reports to their legal teams, consistent with information shared in treatment team meetings.
  • Facilitate assigned groups, including life skills, case management focused topics.
  • Maintain accurate and timely client records to ensure all required documentation is completed.
  • Provide crisis intervention services as needed to support client safety and stability.
  • After a client discharges do a follow up call to check in and share any additional resources within 72 hours of discharge.
  • Establish and maintain positive working relationships with referral sources, community partners, and agency collaborators.
  • Assist clients in arranging appointments and transportation; transport clients when necessary in accordance with agency policy.
  • Proactively engage and communicate with collateral supports, including family members, significant others, probation officers, judges, caseworkers, and other partners.
  • Ensure timely completion of Medicaid applications and address changes for all clients.
  • Document all case management and group services on the same day they are provided.
  • Participate in the Management On-Call schedule as assigned.



Secondary Duties

  • Understand and adhere to 42 CFR pt. 2 Federal Confidentiality and HIPAA laws.
  • Proactively expand and integrate knowledge of community resources.
  • Act and make decisions within the position’s scope of responsibility.
  • Can attend and participate in meetings and training at multiple locations.
  • Learn, utilize, and endorse agency selected clinical techniques, including Motivational Interviewing and Trauma Informed practices.
  • Perform other functions as necessary or assigned.
