As a case manager, you’ll spend time in the community, advocating for individuals living with a mental illness, coordinating care, helping them gain independence and reach their full potential. You will bring care directly to those who need it most by providing hands-on support in real-life settings to help individuals maintain stability, independence, and connection to resources. Meet individuals where they are. Provide advocacy and linkage service in the community. Opportunities available in Burlington and Mercer Counties.
