Case Manager (SCan) – Violence Prevention and Intervention (VPI)

The Puerto Rican Cultural Center was founded in 1973, The Puerto Rican Cultural Center (PRCC) is a non-profit, community-based umbrella institution that seeks to serve the socio-cultural needs of Chicago’s Puerto Rican and Latino community. Position Summary

The Case Manager supports youth at high risk of violence involvement by providing culturally responsive, trauma-informed case management services. This role focuses on building trust, connecting youth to resources, and supporting their progress toward personal, educational, and career goals.

Qualifications:

  • Bachelor’s degree in Social Work or a related field
  • Valid driver’s license and access to a reliable vehicle
  • Strong commitment to working with youth and seeing tasks through to completion
  • Excellent communication, outreach, and relationship-building skills
