Clinical Case Manager (Adoption Support and Preservation)
The Clinical Case Manager – Adoption Support and Preservation (ASAP) program provides healing-centered, family systems-based therapeutic services to adopted children and their families, with the goal of maintaining placement and improving the overall well-being and attachment of the child within the home environment. As a Clinical Case Manager, you will assess the behavioral health needs of participants in community-based and/or grant-funded programs, provide clinical assessments as required by program scope, and provide or recommend appropriate therapeutic interventions.
