Community Support Specialist

The Community Support Specialist provides community support and case coordination services for a caseload of individuals with disabilities/intellectual disabilities or individuals with mental illness. The Community Support Specialists works with the people we serve using a true person center approached in order to foster relationships between individuals and their communities.

-Assess consumer's overall functioning, identifying problems, needs, skill strengths and deficits, and assume a lead role in the consumer centered planning process developing implementation strategies.
-Implement selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports.
-Ensure linkages to community service providers, entitlement programs and housing alternatives.
-Advocate for individual's basic need, development of natural resources, and personal independence through teaching self-advocacy skills.
Minimum requirements include a Bachelor's degree from four-year college or university in the human services field and at least one year of direct care experience working with persons with disabilities. Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to work with persons with developmental disabilities. Must have Mental Health Professional (MHP) status to work with persons with mental illness. Must have reliable transportation, valid driver's license, proof of insurance, and meet company insurability standards by having a good driving record. Employment with Bridgeway is conditional upon completion of all background checks.
