Enrollment and Care Manager

Job Description:


Open Sky Community Services is seeking a compassionate and organized Enrollment and Care Manager to support individuals seeking behavioral health and community-based services through the Rapid Response Hub. This role serves as a key point of contact for individuals, referral sources, and community partners, helping connect people to needed services, navigate barriers to care, and access resources that support recovery and stability. The Enrollment and Care Manager also provides supervision to Targeted Case Managers and helps ensure efficient coordination of referrals, assessments, and service delivery.

Key Responsibilities

  • Review and process referrals, determine eligibility, and connect individuals to appropriate services and supports.

  • Conduct assessments, develop support plans, and monitor individuals' progress throughout the referral and treatment process.

  • Coordinate care with families, providers, community agencies, and treatment partners to promote positive outcomes.

  • Manage program data, documentation, reporting, and grant-related requirements to ensure regulatory compliance.

  • Support clinic operations by coordinating referrals, verifying eligibility, and improving access to services.

  • Provide supervision and guidance to Targeted Case Managers.

Qualifications

  • High school diploma, GED, or equivalent, required.

  • Two years of experience working with individuals experiencing homelessness, mental health challenges, justice involvement, or other underserved populations, required.

  • Knowledge of behavioral health, substance use, and community-based service systems, preferred.

  • Valid driver's license and acceptable driving record, required.


Pay Range:

$25.55-$28.72
