Home Care Scheduling Support Coordinator

At Home Care Partners, the Scheduling Support Coordinator helps keep our caregiving operations running smoothly during afternoons, evenings, weekends, holidays, and other high-need periods. Working alongside the Home Care Schedule Manager and office team, you’ll assist with caregiver communication, open shift outreach, schedule updates, documentation, and after-hours response.
Your work will help reduce coverage gaps, keep caregivers informed, and ensure our clients continue receiving consistent, reliable care.
What You’ll Do (and Why It Matters)
- Support caregiver schedules by assisting with updates, open shifts, and coverage needs
- Contact caregivers about availability, schedule confirmations, weekend commitments, and holiday coverage
- Respond to call-outs, late arrivals, and after-hours scheduling changes
- Communicate schedule updates to caregivers, clients, families, and the office team
- Maintain accurate availability records, scheduling notes, and on-call documentation
- Identify unresolved coverage gaps or reliability concerns and escalate them to the appropriate team member
This role helps keep scheduling organized, communication clear, and client care covered when plans change.
What to Expect from the Role
This is a part-time position with a combination of office hours and remote on-call coverage.
- Approximately 15–25 hours per week
- Weekday hours primarily during afternoons and evenings
- Remote on-call coverage 2–3 weekday evenings per week
- In-office every Saturday from 7:00 AM–2:00 PM
- Weekend on-call coverage two weekends per month
- Rotating holiday on-call coverage
You must be dependable, responsive, and comfortable managing time-sensitive scheduling needs during assigned coverage periods.
