Part Time Special Projects Analyst - SPARC Second Ave

What you will do:

Provides Case Management for the Homelessness Prevention and Rapid Re-Housing Program

Responsibilities:

  • Screens individuals for eligibility in the Rapid Re-housing Program.
  • Determines support services needed and establishes goals with participant to aid in achieving self-sufficiency.
  • Establishes relationships with community organizations and providers through community outreach and follow-up.
  • Establishes and maintains protocols with referents throughout community. Has a thorough understanding of and ability to implement Federal grant processes under guidelines of OASAS and OMH.
  • Maintains documentation according to OASAS, JCAHO, DOH, HUD, and any other designated governing body.
  • Performs other duties as assigned by the Community Support Services Program Manager or the Executive Director.
  • Maintains documentation through word processing and personal computer skills, when applicable.

Requirements:

  • High School Diploma, BA preferred
  • 3-5 years' experience in working with the homeless and exposure to full-spectrum Case Management.

Pay Range: $27.15 - $39.56

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
