Project Coordinator - SBIRT

Horizon is looking for a Project Coordinator at our outpatient clinic located at 699 Hertel Ave, Buffalo NY. The Project Coordinator will become familiar with the SBIRT public health model and the multiple screening tools being used as part of this program, act as a liaison between the schools (or other program settings) and Horizon to implement SBIRT services with adolescent students, and more! This is a full-time position from Monday - Friday. This is a grant-funded position.
