Purpose of Job:
This position is part of a multi-disciplinary team that provides quality patient care in our patient centered medical home model. The purpose of this position is to improve the overall health of the communities we serve, expand the availability of quality health services, and wellness education by coordinating patient referrals at NHP by performing the following duties:

Essential Duties and Responsibilities:

  • Processes referrals in a timely fashion, including sorting and processing referrals by indicated urgency and date.
  • Refers patients to specialists based on each insurance company's available specialty network.
  • Documents and enters all referrals appropriately in patients' medical records.
  • Educates the patients on the referral process including their own responsibilities as well as their insurance company's process and protocols in regards to a referral.
  • Verifies that providers have sufficiently documented the reason for the referral and have done the appropriate work ups prior to sending the patient to the specialist.
  • Maintains referral tracking log per protocol.
  • Performs other duties as assigned.
  • Travel may be required.
