Job Title: Supportive Care (Housing) Case Manager II	Exempt/Non-Exempt: Non-Exempt
Reports to: Program Manager	Full or Part-time: Full-Time
Team: Center for Women and Children
Position Summary:
Is responsible for providing a continuum of care for guests of Wheeler Mission through case management, involving a thorough assessment and identification of each guest's resources, talents, skills and interests including a plan to identify and achieve personal goals. Key Duties of this position include general case management activities, mentoring, discipleship, counseling, as well as teaching and managing groups when needed.
