Targeted Case Manager - CCBHC

The Targeted Case Manager (TCM) is responsible for coordinating and assuring the continuity of services for patients. The TCM will provide outreach to patients coming from a higher level of care to engage them, provide information about their aftercare visit, and remove barriers to participating in services. This is a full-time position from Monday-Friday.
